Teacher Parent Questionnaires

This packet contains 2 different questionnaires. One intended for parents (a question bank) and a complete survey form ready to send to teachers (focusing on Common Core Speaking and Listening Skills) regarding the communication skills of a student. Make sure you are asking the right questions! It is imperative to gather data from both the parents and teachers (multiple contexts) specifically as it relates to the communication skills in the home and classroom environments. Many questionnaires used by SLPs ask only basic informational questions that may not have educational and communication relevancy. These questions will not only be informative in assisting with background knowledge and educational planning, but, also identifying areas of potential strengths and difficulties that may not come to light should questions contained in these products not be asked.
